A CCRC, or Life Plan Community, is a residential community for older adults that provides a variety of living options, services, amenities, and a continuum of care designed to support changing needs over time.

Luther Oaks offers independent living, assisted living, memory support, skilled nursing, and rehabilitation services on one campus.

Prior to admission, a professional clinical assessment is completed to help ensure successful daily living and to develop a care plan based on individual needs.

Support may include medication reminders, transportation, bathing or showering, dressing, escorts for meals and activities, eating assistance, toileting and incontinence care, overnight checks, injections, and wound care.

Memory support can be helpful when concerns include personal safety, anxiety, nutrition, medication management, and the challenges that can come with increasing cognitive decline.

Skilled nursing is the right choice for those who require attentive medical care and a higher level of support than assisted living provides, including long-term needs and rehabilitation support.

Short-term rehabilitation supports residents recovering from hospital stays, illness, or injury with therapy services designed to help them reach maximum function and return to their homes in the community.

Outpatient therapy is available by appointment to the general public on-site at Luther Oaks. Short-term rehabilitation refers to therapy services available to Luther Oaks residents during recovery.
